DLL4-targeted CAR-T is a preclinical chimeric antigen receptor (CAR) T-cell therapy designed to target delta-like ligand 4 (DLL4). Developed by researchers at Tianjin Medical University, this therapy is designed to eliminate DLL4-positive tumor cells, which are associated with cancer stemness and resistance to neoadjuvant chemotherapy (specifically the trastuzumab, pertuzumab, and paclitaxel [THP] regimen) in HER2-positive breast cancer. Mechanistically, DLL4 expression on tumor cells promotes stemness and induces the formation of neutrophil extracellular traps (NETs) via Notch signaling, leading to T-cell exclusion and chemotherapy resistance. By targeting and eliminating DLL4-positive cells, this CAR-T therapy aims to reverse resistance and sensitize tumors to neoadjuvant chemotherapy.
