DMSO + heparin + hydrocortisone + bupivacaine is an intravesical combination therapy, often referred to as a 'DMSO cocktail,' developed by Assaf-Harofeh Medical Center for the treatment of interstitial cystitis and painful bladder syndrome. The therapy is administered via weekly instillations into the bladder. Dimethyl sulfoxide (DMSO) serves as the primary active ingredient and carrier, providing anti-inflammatory and analgesic effects while enhancing the penetration of other components. Heparin sulfate is included to help restore the protective glycosaminoglycan (GAG) layer of the bladder wall. Hydrocortisone provides additional corticosteroid-mediated anti-inflammatory action, and bupivacaine acts as a local anesthetic to provide immediate symptomatic relief from bladder pain. This multi-modal approach targets the underlying inflammation, epithelial dysfunction, and sensory nerve activation associated with chronic bladder pain conditions.
