DN4.99 TCR-T cells are an experimental adoptive cell therapy consisting of human T cells engineered to express a T-cell receptor (TCR) specific for the lipid antigen methyl-lysophosphatidic acid (mLPA) presented by the monomorphic CD1c molecule. Developed by researchers at the San Raffaele Scientific Institute, this therapy targets hematological malignancies such as acute myeloid leukemia (AML) and B-cell acute lymphoblastic leukemia (B-ALL). Unlike traditional MHC-restricted TCR-T therapies, DN4.99 targets CD1c, which is non-polymorphic, potentially allowing for a "universal" donor-unrestricted application across different patients without the need for HLA matching. Preclinical studies demonstrate that these cells selectively kill leukemia blasts while sparing normal CD1c-expressing cells like B cells and monocytes, due to the high enrichment of mLPA in malignant cells.
