Overview

The DNAJB1-PRKACA peptide vaccine is a therapeutic cancer vaccine designed to induce an immune response against tumors harboring the DNAJB1-PRKACA gene fusion. This gene fusion is the oncogenic driver in fibrolamellar hepatocellular carcinoma (FL-HCC) and has also been identified in other tumor types such as oncocytic neoplasms of the pancreas and bile ducts. The vaccine consists of synthetic peptides corresponding to neoepitopes created by the unique junction region of the chimeric protein produced by this gene fusion. These neoepitopes are presented on tumor cells via HLA molecules and can be recognized by T cells, enabling targeted immunotherapy. The mechanism involves priming tumor-specific T-cell responses against these neoepitopes. The approach is often combined with immune checkpoint inhibitors (such as atezolizumab, nivolumab, or ipilimumab) to enhance anti-tumor immunity by overcoming inhibitory signals within the tumor microenvironment[2][4][5]. Early clinical trials have demonstrated safety and induction of robust T cell responses in a subset of patients[5][7].
