DNV3837 is an intravenous, narrow-spectrum, synthetic antibiotic that acts as a prodrug of DNV3681 (also known as MCB3681), a hybrid oxazolidinone-quinolone compound. Upon administration, it is converted *in vivo* to its active form (DNV3681), which crosses the gastrointestinal barrier and accumulates in the intestinal lumen. This allows for targeted treatment of *Clostridioides difficile* infections (CDI) by delivering high concentrations directly to the infection site while sparing the gut microbiota. The drug demonstrates potent activity against Gram-positive bacteria, including hypervirulent strains of *C. difficile*, and has shown superiority over reference treatments such as fidaxomicin in preclinical studies and early clinical trials. It is being developed as a first-line therapy for CDI and has received FDA Qualified Infectious Disease Product (QIDP) designation and Fast Track status[1][2][4][5][7].
