DNX-2401 + pembrolizumab is a combination therapy under investigation for the treatment of recurrent glioblastoma and gliosarcoma. DNX-2401 is a genetically engineered oncolytic adenovirus designed to selectively infect and lyse tumor cells while sparing normal tissue. Its intratumoral administration aims to directly destroy cancer cells and stimulate an anti-tumor immune response by releasing tumor antigens. Pembrolizumab is a monoclonal antibody that targets the programmed cell death protein 1 (PD-1) receptor on T-cells, blocking its interaction with PD-L1/PD-L2 ligands and thereby enhancing T-cell-mediated immune responses against tumors. The rationale for combining these agents is that viral lysis of tumor cells by DNX-2401 may prime the immune system, increasing PD-1 expression in the tumor microenvironment and making subsequent checkpoint inhibition with pembrolizumab more effective[1][3][4]. This combination has shown promising safety and efficacy signals in early-phase clinical trials for patients with recurrent glioblastoma who have failed standard therapies[1][6].
