Dobesilic acid is an organic compound classified as both a phenol and a sulfonic acid, with the molecular formula C6H6O5S. It serves as the parent compound for several pharmaceutical salts. The most clinically relevant derivative is calcium dobesilate, which acts as a vasoprotective agent used in the treatment of chronic venous disease, diabetic retinopathy, hemorrhoidal disease, and other microangiopathies. Dobesilic acid and its salts work primarily by reducing capillary permeability through stabilization of collagen chains in the basement membrane of blood vessels and interaction with biochemical mediators that regulate endothelial permeability. This leads to improved microcirculatory function, reduced blood hyperviscosity, and anti-platelet aggregation effects. The drug is generally well absorbed orally (as calcium dobesilate), has low plasma protein binding, minimal metabolism (<10%), and is mainly excreted renally.
