Overview

The PLATI-PARP regimen is a sequential therapeutic strategy developed by the University of Washington and Fred Hutchinson Cancer Center for the treatment of metastatic castration-resistant prostate cancer (mCRPC) in patients with homologous recombination repair (HRR) gene deficiencies. The regimen consists of an induction phase of chemotherapy using docetaxel and carboplatin, followed by a maintenance phase with the PARP inhibitor rucaparib. Docetaxel acts by stabilizing microtubules to inhibit cell division, while carboplatin creates DNA cross-links that cause structural damage. Rucaparib subsequently targets poly (ADP-ribose) polymerase (PARP) enzymes, preventing the repair of single-strand DNA breaks. In cells with existing HRR defects, this combination leads to the accumulation of double-strand breaks and selective tumor cell death through synthetic lethality.
