Overview

This is a four-drug combination regimen consisting of docetaxel, carboplatin, trastuzumab, and bevacizumab. It has been investigated primarily for the treatment of HER2-positive breast cancer, including early-stage disease and metastatic settings. - **Docetaxel** is a taxoid antineoplastic agent that stabilizes microtubules by binding to the β-subunit of tubulin, preventing their depolymerization and thereby inhibiting cell division[5]. - **Carboplatin** is a platinum-based chemotherapeutic that forms DNA crosslinks leading to apoptosis in rapidly dividing cells. - **Trastuzumab** is a monoclonal antibody targeting the human epidermal growth factor receptor 2 (HER2), inhibiting proliferation of tumor cells that overexpress HER2. - **Bevacizumab** is a recombinant humanized monoclonal antibody against vascular endothelial growth factor A (VEGF-A), blocking angiogenesis by preventing VEGF from binding its receptors on endothelial cells[6]. The combination aims to target both tumor cell proliferation (via cytotoxic chemotherapy and anti-HER2 therapy) and tumor angiogenesis (via anti-VEGF therapy). Clinical studies have shown activity in HER2-positive breast cancer; however, addition of bevacizumab did not significantly improve overall survival but increased toxicity[1][2].
