This combination therapy consists of the immune checkpoint inhibitor avelumab integrated with a modified docetaxel, cisplatin, and 5-fluorouracil (mDCF) chemotherapy regimen. Avelumab is a human IgG1 monoclonal antibody that targets programmed death-ligand 1 (PD-L1), preventing it from binding to PD-1 and B7.1 receptors on T cells, thereby restoring anti-tumor immune responses. The mDCF component provides cytotoxic debulking: docetaxel stabilizes microtubules to inhibit mitosis, cisplatin induces DNA cross-linking to trigger apoptosis, and 5-fluorouracil inhibits thymidylate synthase to disrupt DNA synthesis. This perioperative regimen is primarily being investigated for the treatment of resectable locally advanced esophago-gastric adenocarcinomas to improve pathologic complete response rates and disease-free survival.
