TFOX is a triplet chemotherapy regimen consisting of docetaxel, 5-fluorouracil (5-FU), oxaliplatin, and folinic acid (leucovorin). It is a modified version of the FLOT regimen, typically utilizing a lower dose of docetaxel (50 mg/m²) to improve tolerability while maintaining clinical efficacy in the first-line metastatic setting. TFOX is primarily investigated for the treatment of advanced, HER2-negative gastric or gastro-oesophageal junction adenocarcinoma. In the Phase III PRODIGE 51-FFCD-GASTFOX trial, TFOX demonstrated superior progression-free survival and overall survival compared to the doublet FOLFOX regimen, although it was associated with a higher incidence of grade 3-4 toxicities, including diarrhea, neutropenia, and peripheral neuropathy.
