Overview

This is a sequential doublet chemotherapy regimen consisting of docetaxel and methotrexate followed by gemcitabine and cisplatin. It was evaluated in a Phase II clinical trial led by the University of Chicago for the treatment of patients with previously untreated stage IV locally advanced or metastatic bladder cancer (urothelial carcinoma). The rationale behind this sequential approach is to maximize tumor cell kill by using non-cross-resistant agents in a specific sequence. Gemcitabine, a nucleoside analog, and cisplatin, a platinum-based alkylating agent, are standard components of bladder cancer therapy, while docetaxel (a taxane) and methotrexate (an antifolate) provide additional cytotoxic mechanisms. The trial aimed to improve outcomes in advanced bladder cancer by alternating these two doublets.
