This is a four-drug combination regimen consisting of docetaxel, oxaliplatin, capecitabin (capecitabine), and bevacizumab. Docetaxel is a taxane-class chemotherapeutic agent that inhibits microtubule depolymerization, leading to cell cycle arrest and apoptosis. Oxaliplatin is a platinum-based chemotherapeutic that forms DNA crosslinks, inhibiting DNA replication and transcription. Capecitabine is an oral prodrug of 5-fluorouracil (5-FU) that interferes with DNA synthesis by inhibiting thymidylate synthase. Bevacizumab is a monoclonal antibody targeting vascular endothelial growth factor A (VEGF-A), thereby inhibiting angiogenesis in tumors. The combination has been studied as first-line therapy for advanced HER2-negative gastric or gastroesophageal adenocarcinoma, showing feasibility and activity with notable progression-free survival and overall survival outcomes[1][2][8]. The regimen may be followed by maintenance therapy with capecitabine and bevacizumab in patients achieving disease control[1].
