PISOXO is an experimental neoadjuvant combination therapy regimen developed by Ruijin Hospital for the treatment of locally-invaded gastric cancer (LIGC). The regimen integrates three distinct therapeutic modalities: Pressurized Intra Peritoneal Aerosol Chemotherapy (PIPAC) using docetaxel to target peritoneal disease, systemic SOX chemotherapy (comprising intravenous oxaliplatin and the oral fluoropyrimidine S-1, which contains tegafur, gimeracil, and oteracil), and the PARP inhibitor olaparib. This multi-modal approach is designed to provide both localized control of peritoneal micrometastases and systemic control of the primary tumor and distant disease. Olaparib is included to potentially exploit DNA repair deficiencies in gastric cancer cells, thereby enhancing the cytotoxic effects of the platinum-based chemotherapy (oxaliplatin) through synthetic lethality or potentiation of DNA damage.
