docetaxel + PI-88 is an investigational combination therapy evaluated for advanced cancers, notably androgen-independent (castrate-resistant) prostate cancer and advanced solid malignancies. Docetaxel is an approved chemotherapeutic agent that inhibits microtubule depolymerization, inducing apoptosis in cancer cells. PI-88 is a novel, sulfated oligosaccharide that functions as a multi-targeted heparanase inhibitor and angiogenesis inhibitor by blocking the degradation of heparan sulfate, preventing growth factor release from the extracellular matrix, and inhibiting blood vessel formation critical for tumor growth and metastasis. The combination is studied due to potential synergistic efficacy; however, hematologic toxicities, particularly febrile neutropenia, have limited further development at initially tested doses[1][3][5].
