Docosahexaenoic acid (DHA) is a long-chain omega-3 polyunsaturated fatty acid being investigated by the Fundació Institut de Recerca de l'Hospital de la Santa Creu i Sant Pau for the treatment of metabolic complications in HIV-infected patients. Specifically, it targets hypertriglyceridemia, hypercholesterolemia, and lipodystrophy associated with highly active antiretroviral therapy (HAART). DHA acts as an agonist of the nuclear receptor Peroxisome Proliferator-Activated Receptor gamma (PPAR-gamma), which promotes adipocyte differentiation and modulates lipid metabolism. It also exhibits anti-inflammatory properties, potentially reducing TNF-alpha levels. In clinical trials, high-dose DHA (4g daily) significantly reduced triglyceride levels in patients on combination antiretroviral therapy.
