dofetilide

Dofetilide is a class III antiarrhythmic agent used to treat irregular heartbeats, specifically atrial fibrillation and atrial flutter. It works by selectively blocking the rapid component of the delayed rectifier outward potassium current (IKr), which increases the refractory period of atrial tissue and helps restore and maintain normal sinus rhythm. Dofetilide does not affect conduction velocity or resting membrane potential. The drug is available as an oral capsule and requires close monitoring during initiation due to its potential to cause serious arrhythmias, including QT prolongation and torsade de pointes. Dofetilide is marketed under the brand name Tikosyn in the United States.
