dolutegravir + darunavir + ritonavir (ANRS)

Dolutegravir + darunavir + ritonavir is a third-line antiretroviral therapy (ART) regimen evaluated by ANRS, Emerging Infectious Diseases, specifically in the ANRS 12369 THILCO trial in Cambodia. This regimen is intended for HIV-1 infected adults who have failed second-line protease inhibitor-based treatments. It consists of dolutegravir (an integrase strand transfer inhibitor), darunavir (a protease inhibitor), and ritonavir (a pharmacokinetic booster), often combined with optimized nucleoside reverse transcriptase inhibitors (NRTIs) like lamivudine. Dolutegravir prevents the integration of viral DNA into the host cell genome by inhibiting the integrase enzyme. Darunavir binds to the active site of the HIV-1 protease, preventing the processing of viral polyproteins into functional proteins, which results in the formation of immature, non-infectious virions. Ritonavir is used at a low dose to inhibit the CYP3A4 enzyme, thereby increasing the systemic exposure and half-life of darunavir.
