This combination is a multi-drug antiretroviral therapy for HIV infection, comprised of four distinct small-molecule drugs: **dolutegravir** (an integrase strand transfer inhibitor), **etravirine** (a non-nucleoside reverse transcriptase inhibitor), **darunavir** (a protease inhibitor), and **ritonavir** (a protease inhibitor and pharmacokinetic booster). Dolutegravir blocks HIV integrase, preventing the integration of viral DNA into the host genome. Etravirine inhibits HIV reverse transcriptase by binding non-competitively. Darunavir inhibits the viral protease, preventing the proteolytic cleavage of viral polyproteins, while ritonavir primarily acts as a booster by inhibiting cytochrome P450 3A4, thereby raising levels of darunavir and other co-administered drugs. Clinical pharmacokinetic data demonstrate that etravirine can decrease dolutegravir concentrations, but this effect is mitigated by coadministration with darunavir/ritonavir; thus, this combination may be used together without dose adjustment, typically in salvage or second-line therapy for HIV-1 infection[1][3][4][5].
