Donor-Derived Ex-Vivo Expanded Natural Killer Cell refers to a cellular immunotherapy product consisting of natural killer (NK) cells obtained from a healthy donor, which are then expanded and activated outside the body (ex vivo) using specialized culture systems—often involving cytokines such as interleukin-21 or feeder cell lines expressing stimulatory molecules. These NK cells are typically derived from peripheral blood or cord blood of HLA-haploidentical donors and are administered to patients, often after lymphodepleting chemotherapy or hematopoietic stem cell transplantation. The primary mechanism is direct cytotoxicity against malignant cells, with the goal of enhancing immune reconstitution, reducing relapse rates, and decreasing infectious complications in high-risk hematologic malignancies such as acute myeloid leukemia. Clinical trials have demonstrated feasibility and safety for this approach, with no significant increase in graft-versus-host disease[1][3][8].
