Overview

Donor-derived WT1-specific cytotoxic T lymphocytes (CTLs) is an investigational adoptive cell-based immunotherapy developed by the University of Sydney for the prevention of relapse in patients with acute myeloid leukemia (AML) undergoing allogeneic hematopoietic stem cell transplantation (HSCT). The therapy consists of T cells harvested from the transplant donor, which are expanded ex vivo and selected for specificity against the Wilms' tumor 1 (WT1) antigen using CD137+ selection. WT1 is a tumor-associated antigen that is highly overexpressed in myeloid malignancies but has limited expression in healthy tissues, making it a suitable target for post-transplant immunotherapy. In clinical trials such as INTACT-TAA (NCT02895412), these WT1-specific CTLs are administered in a prophylactic regimen, often following the infusion of pathogen-specific T cells (P-CTL), to provide dual protection against leukemic recurrence and opportunistic infections.
