Overview

Donor gamma-delta T cells are a **cell therapy product** composed of gamma-delta (γδ) T cells, a distinct subset of T lymphocytes obtained from healthy donors (allogeneic). These cells differ from conventional alpha-beta T cells by expressing the gamma-delta T-cell receptor (TCR) and exhibit both innate and adaptive immune properties. Donor γδ T cells are typically expanded ex vivo from donor peripheral blood mononuclear cells (PBMCs) using agents such as zoledronate (a bisphosphonate) in combination with cytokines (notably interleukin-2, IL-2). Once expanded, these cells are reinfused into patients, most commonly for immuno-oncology indications. The mechanism of action involves **direct cytotoxicity toward malignant or infected cells**, typically independent of human leukocyte antigen (HLA) restriction, lowering the risk of graft-versus-host disease (GvHD) and making these cells suitable for allogeneic (donor-based) approaches in adoptive cell transfer (ACT). Major clinical applications explored are in **hematological malignancies** and **solid tumors**, with ongoing development of both unmodified and genetically modified (e.g., CAR-transfected) donor γδ T cell products.
