Overview

Donor lymphocyte infusion (DLI) is an adoptive cellular immunotherapy used to induce or maintain remission after allogeneic hematopoietic stem cell transplantation. In the specific protocol developed by the Cancer and Leukemia Group B (CALGB 109901), DLI was utilized as a secondary intervention for patients with progressive disease following a reduced-intensity conditioning transplant for B-cell malignancies, including chronic lymphocytic leukemia (CLL), prolymphocytic leukemia (PLL), non-Hodgkin lymphoma (NHL), and mantle cell lymphoma (MCL). The therapy involves the infusion of CD3+ T cells from the original stem cell donor. These lymphocytes recognize and attack residual malignant cells through the graft-versus-leukemia (GVL) effect, leveraging the donor's immune system to achieve long-term disease control. The CALGB 109901 trial specifically combined this with a conditioning regimen of fludarabine and cyclophosphamide.
