Donor lymphocyte infusion (DLI) is a cellular immunotherapy protocol developed by Nanfang Hospital, Southern Medical University, specifically designed to prevent or treat relapse in patients with acute leukemia following allogeneic hematopoietic stem cell transplantation (allo-HSCT). This approach involves the infusion of lymphocytes collected from the original stem cell donor, often mobilized with G-CSF and cryopreserved for later use. The protocol utilizes a minimal residual disease (MRD)-directed and graft-versus-host disease (GVHD)-guided strategy to optimize the graft-versus-leukemia (GVL) effect while minimizing the risk of severe GVHD. By introducing donor-derived T cells, the therapy aims to eliminate residual leukemic cells that survived the initial transplant conditioning and graft, thereby reducing the incidence of hematologic relapse.
