Overview

Donor lymphocyte infusion (DLI) is a cellular immunotherapy procedure being investigated by the National Cancer Institute (NCI) for the treatment of high-risk hematologic malignancies. In the context of the Phase I/II trial NCT05327023, DLI involves the infusion of peripheral blood lymphocytes (specifically CD3+ T cells) from the original hematopoietic stem cell transplant donor into the recipient early after an allogeneic hematopoietic cell transplant. This procedure aims to induce a graft-versus-leukemia (GVL) effect, where the donor's immune cells recognize and eliminate residual malignant cells in the recipient. The trial specifically evaluates the safety and efficacy of prophylactic DLI administered following a conditioning regimen that includes post-transplantation cyclophosphamide (PTCy) to mitigate graft-versus-host disease (GVHD) while preserving the anti-tumor effect.
