Overview

Donor lymphocyte infusion (DLI) is a form of adoptive cellular immunotherapy used primarily to treat relapsed hematologic malignancies following allogeneic hematopoietic stem cell transplantation (HSCT). In this specific National Cancer Institute (NCI) pilot study (NCT00984165), DLI is combined with a single fraction of low-dose radiation (8 Gy) to enhance the graft-versus-tumor (GVT) effect. The therapy involves the infusion of unmanipulated lymphocytes, primarily T cells, collected from the original stem cell donor through apheresis. These donor T cells recognize and attack recipient tumor cells by identifying minor histocompatibility antigens or tumor-associated antigens. The radiation component is hypothesized to sensitize tumor cells and promote the release of antigens, thereby augmenting the donor immune response against Hodgkin's lymphoma, non-Hodgkin's lymphoma, chronic lymphocytic leukemia, and multiple myeloma.
