Donor lymphocyte infusion (DLI) is an adoptive cell therapy that involves the administration of mature lymphocytes from an allogeneic donor to a transplant recipient. In the specific clinical program led by Wake Forest University Health Sciences, DLI is utilized to accelerate immune reconstitution in patients who have undergone T-cell depleted (CD34+ selected) hematopoietic stem cell transplantation. The infused donor T cells are intended to restore cellular immunity, providing protection against opportunistic infections and mediating a graft-versus-leukemia (GVL) effect to prevent disease relapse. This therapeutic approach is often combined with methotrexate to provide prophylaxis against graft-versus-host disease (GVHD), a common complication of DLI.
