Donor-specific immunomodulatory cells (DISIC) are an investigational cell therapy developed by Karolinska University Hospital to promote immune tolerance in patients undergoing liver transplantation for end-stage liver disease. The therapy consists of donor-derived leucocytes that are processed to induce a tolerogenic phenotype. When infused into the recipient, these cells are intended to suppress the host's anti-donor immune response and stimulate the expansion of recipient regulatory T cells (Tregs). The ultimate clinical goal of DISIC therapy is to achieve operational tolerance, allowing for the significant reduction or complete withdrawal of standard maintenance immunosuppressive drugs, thereby avoiding their long-term side effects such as nephrotoxicity and increased infection risk.
