Donor T-cell adback is a cellular immunotherapy protocol developed and evaluated by the M.D. Anderson Cancer Center for patients with lymphoid malignancies, such as leukemia and lymphoma. This therapeutic approach involves the infusion of T-lymphocytes derived from an allogeneic donor into a recipient following an allogeneic stem cell transplant (allo-SCT). It is specifically designed for use in patients who have undergone conditioning regimens containing T-cell-depleting agents like alemtuzumab (Campath-1H). The primary objective of the "adback" strategy is to induce a graft-versus-tumor (GVT) effect, where the donor's immune cells recognize and eliminate residual malignant cells in the recipient. Additionally, these cells help to restore immune competence and facilitate full donor cell engraftment, counteracting the profound T-cell depletion caused by the conditioning regimen while carefully monitoring for graft-versus-host disease (GVHD).
