Overview

Doravirine is a next-generation non-nucleoside reverse transcriptase inhibitor (NNRTI) developed by Merck for the treatment of HIV-1 infection. It functions by binding allosterically to the HIV-1 reverse transcriptase enzyme, thereby inhibiting the conversion of viral RNA into DNA. Unlike earlier NNRTIs, doravirine maintains significant activity against common NNRTI-resistant mutations such as K103N, Y181C, and G190A. It is FDA-approved as a standalone 100 mg tablet (Pifeltro) and as part of a fixed-dose combination (Delstrigo). The University of North Carolina, Chapel Hill is currently conducting a Phase 1 pharmacokinetic study (NCT04900974) to evaluate the appropriate dosing of doravirine in pregnant women with HIV during the second and third trimesters and the postpartum period.
