Dorsal penile block anesthesia is a regional anesthetic technique being evaluated by McMaster University in the CLASH (Caudal vs Local Anesthesia in Hypospadias) trial. The procedure involves the infiltration of a local anesthetic, specifically 0.25% bupivacaine without epinephrine, at the 10 and 2 o'clock positions at the base of the penis to block the dorsal nerves. This technique is primarily used for perioperative analgesia in pediatric patients undergoing hypospadias repair. The McMaster study (NCT02512887) aims to determine if this localized approach results in lower rates of postoperative surgical complications, such as urethrocutaneous fistulas and glans dehiscence, compared to the more traditional caudal epidural block.
