Overview

This is a **combination regimen** consisting of four agents: - **Dostarlimab** is a humanized monoclonal antibody that acts as a **PD-1 immune checkpoint inhibitor**. It blocks the programmed death-1 (PD-1) receptor on T-cells, enabling enhanced immune recognition and killing of tumor cells. - **Carboplatin** is a platinum-based antineoplastic agent that forms DNA adducts, leading to apoptosis and inhibition of cell replication. - **Paclitaxel** is a taxane-class chemotherapeutic that stabilizes microtubules, disrupting mitosis and leading to cell death. - **Niraparib** is a **PARP inhibitor** (poly ADP ribose polymerase), which impedes DNA repair in tumor cells—especially effective in tumors with homologous recombination deficiencies such as BRCA mutations. This regimen is used experimentally or investigationally for **gynecologic cancers**, especially **advanced or recurrent endometrial and ovarian cancer**. Dostarlimab in combination with carboplatin and paclitaxel is FDA-approved for advanced/recurrent endometrial cancer, but the addition of niraparib is not a standard, widely approved regimen; trials are ongoing to determine the efficacy and safety of the four-drug combination.
