Overview

DOTANOC is a synthetic somatostatin analog peptide, specifically DOTA-(1-Nal)3-octreotide, which acts as a ligand for somatostatin receptors. It is commonly used as a precursor for radiolabeling with radionuclides such as gallium-68 (Ga-68), creating the radiopharmaceutical Ga-68-DOTANOC. This compound binds to multiple subtypes of somatostatin receptors (SSTRs), particularly SSTR2, SSTR3, and SSTR5, making it a pan-somatostatin analog. When labeled with Ga-68 and administered intravenously, it enables positron emission tomography/computed tomography (PET/CT) imaging for the localization and management of neuroendocrine tumors (NETs). DOTANOC PET/CT has demonstrated high sensitivity and specificity in detecting both primary NETs and metastases by targeting overexpressed somatostatin receptors on tumor cells[7][8][9]. The broad receptor affinity profile distinguishes DOTANOC from other similar agents like DOTATATE or DOTATOC.
