Overview
Dotinurad + allopurinol is a combination therapy under investigation for the treatment of hyperuricemia and gout. Dotinurad is a selective urate reabsorption inhibitor that lowers serum uric acid levels by inhibiting the URAT1 transporter in the kidneys, thereby increasing urinary excretion of urate[7][8]. Allopurinol is a xanthine oxidase inhibitor that reduces the production of uric acid by blocking its synthesis from hypoxanthine and xanthine[2][6]. The combination aims to provide additive or synergistic effects on lowering serum uric acid through dual mechanisms—decreasing production (allopurinol) and increasing excretion (dotinurad). This approach may benefit patients with gout or hyperuricemia who are inadequately controlled on monotherapy. Clinical studies are ongoing to evaluate pharmacokinetics, pharmacodynamics, safety, and efficacy of this combination in patients with gout and hyperuricemia[3].
