Double Umbilical Cord Blood Transplantation (dUCBT) is an investigational cell-based therapy being evaluated by the Tehran University of Medical Sciences (notably in trial NCT01015742) for the treatment of high-risk hematologic malignancies, including acute myeloid leukemia (AML), acute lymphoblastic leukemia (ALL), and chronic myeloid leukemia (CML). This procedure involves the infusion of two unrelated umbilical cord blood units to provide a sufficient dose of hematopoietic stem cells, which is particularly beneficial for adult patients who lack a single high-dose unit or an HLA-matched donor. The protocol includes a conditioning regimen of busulfan, cyclophosphamide, and antithymocyte globulin (ATG) to ablate the recipient's bone marrow and prevent graft rejection, followed by graft-versus-host disease (GVHD) prophylaxis using cyclosporin and methylprednisolone. The therapy aims to achieve full hematopoietic reconstitution and leverage the graft-versus-leukemia effect to prevent disease relapse.
