Doxapram is a short-acting respiratory stimulant (analeptic) used primarily to treat acute respiratory insufficiency in patients with chronic obstructive pulmonary disease (COPD), as well as to stimulate respiration in cases of drug-induced postanesthetic respiratory depression or central nervous system depression due to drug overdose. It is administered intravenously and acts rapidly, increasing tidal volume and slightly increasing the respiratory rate. Doxapram’s mechanism of action involves stimulation of peripheral carotid chemoreceptors by inhibiting specific potassium channels—primarily TASK-1 and TASK-3 channels—which leads to depolarization and increased neurotransmitter secretion that stimulates the respiratory center. The drug may also cause a pressor response due to improved cardiac output rather than peripheral vasoconstriction. Doxapram does not antagonize the analgesic effects of opioids but can reverse opioid-induced respiratory depression[1][3][6][8].
