Overview

PIPAC (Pressurized Intraperitoneal Aerosol Chemotherapy) is an innovative drug delivery method used to treat peritoneal malignancies. In this specific application developed and studied by Assistance Publique - Hôpitaux de Paris (AP-HP) for gastric adenocarcinoma with peritoneal carcinomatosis, a combination of doxorubicin and cisplatin is administered as a pressurized aerosol into the peritoneal cavity during a laparoscopic procedure. Doxorubicin is an anthracycline that inhibits topoisomerase II and intercalates DNA, while cisplatin is a platinum-based agent that forms DNA cross-links, both leading to apoptosis in rapidly dividing cancer cells. This approach aims to improve drug penetration into peritoneal nodules compared to conventional systemic chemotherapy while minimizing systemic toxicity. The regimen is typically repeated every 4 to 6 weeks and is evaluated in combination with systemic chemotherapy.
