AC-T is a standard-of-care combination chemotherapy regimen widely used in the neoadjuvant and adjuvant treatment of early-stage and advanced breast cancer, particularly high-risk or node-positive disease. The regimen is administered sequentially, typically beginning with four cycles of AC (doxorubicin [Adriamycin], an anthracycline topoisomerase II inhibitor, and cyclophosphamide [Cytoxan], an alkylating agent) followed by four cycles of T (paclitaxel [Taxol], a microtubule-stabilizing taxane, or sometimes docetaxel [Taxotere]). AC-T works by targeting rapidly dividing cancer cells through multiple complementary mechanisms: doxorubicin intercalates DNA and inhibits topoisomerase II, cyclophosphamide induces DNA cross-linking, and paclitaxel prevents microtubule depolymerization, collectively arresting the cell cycle and inducing apoptosis.
