Overview

Doxorubicin + cyclophosphamide (AC) or docetaxel + cyclophosphamide (TC) refers to a treatment paradigm evaluated in the neoadjuvant setting for HER2-negative early-stage breast cancer. The AC regimen combines doxorubicin, an anthracycline that inhibits topoisomerase II and intercalates DNA, with cyclophosphamide, an alkylating agent that cross-links DNA strands. The TC regimen combines docetaxel, a taxane that stabilizes microtubules to inhibit mitosis, with cyclophosphamide. This specific "either/or" approach was the focus of a Duke University-sponsored Phase II trial (NCT00636441), which investigated whether genomic expression profiling could guide the selection between these two regimens to optimize the pathologic complete response rate in patients.
