AD + CP is a combination chemotherapy regimen comprising doxorubicin (Adriamycin), dacarbazine, cyclophosphamide, and prednisone. It is primarily utilized as a reduced-intensity chemotherapy backbone in clinical trials for pediatric and young adult patients with classical Hodgkin lymphoma (cHL). In the Phase 2 RADICAL trial (NCT05439239) led by New York Medical College, AD + CP is administered in combination with the PD-1 inhibitor nivolumab. The goal of this combination is to maintain high cure rates while reducing the cumulative dose of anthracyclines and avoiding the need for radiation therapy, thereby minimizing long-term toxicities in younger populations. Doxorubicin acts as a DNA intercalator and topoisomerase II inhibitor, dacarbazine and cyclophosphamide function as DNA alkylating agents, and prednisone is a corticosteroid that provides lymphocytotoxic effects.
