This drug is a combination chemotherapy regimen consisting of three active agents: doxorubicin, methotrexate, and vinorelbine. Each component has distinct mechanisms of action that contribute to the overall antineoplastic effect. - **Doxorubicin** is an anthracycline antibiotic that intercalates DNA strands, inhibits topoisomerase II enzyme activity, and generates free radicals leading to DNA damage and apoptosis in cancer cells. - **Methotrexate** is an antimetabolite and antifolate agent that inhibits dihydrofolate reductase (DHFR), disrupting folic acid metabolism necessary for DNA synthesis and cell replication. - **Vinorelbine** is a semi-synthetic vinca alkaloid that disrupts microtubule formation by binding tubulin, thereby inhibiting mitosis. The combination leverages different cytotoxic mechanisms targeting cancer cells at various points in the cell cycle. It has been studied primarily for treatment of cancers such as desmoid fibromatosis and urothelial carcinoma with evidence supporting efficacy when used together at specific doses. The regimen can be administered with leucovorin rescue to mitigate methotrexate toxicity.
