Doxycycline + levofloxacin is a combination antibiotic regimen being evaluated for the treatment of brucellosis, including both uncomplicated and osteoarticular forms. This dual therapy serves as a rifampin-sparing alternative to the standard World Health Organization (WHO) recommended doxycycline-rifampin regimen, aiming to reduce the selection pressure for rifampin resistance. Doxycycline is a tetracycline antibiotic that inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit. Levofloxacin is a third-generation fluoroquinolone that inhibits bacterial DNA gyrase and topoisomerase IV, thereby preventing DNA replication and transcription. The combination is particularly noted for the superior bone and joint tissue penetration of levofloxacin, which is advantageous in treating complicated osteoarticular brucellosis lesions.
