DR5-B is an engineered recombinant protein variant of the Tumor Necrosis Factor-Related Apoptosis-Inducing Ligand (TRAIL), specifically designed for high selectivity toward Death Receptor 5 (DR5, also known as TRAIL-R2). Unlike wild-type TRAIL, which binds to both Death Receptor 4 (DR4) and DR5 as well as decoy receptors, DR5-B is mutated to selectively activate the pro-apoptotic signaling of DR5. This selectivity is intended to enhance its therapeutic index by reducing off-target binding and potential toxicity. DR5-B has been investigated in preclinical research for its efficacy in treating solid tumors, particularly pancreatic adenocarcinoma. It is frequently studied in combination with sensitizing agents like the proteasome inhibitor bortezomib and incorporated into advanced delivery systems, such as amphiphilic poly(N-vinylpyrrolidone) biocomposites, to improve its stability and internalization in the tumor microenvironment.
