The DRibble vaccine is an autologous, autophagosome-based therapeutic cancer vaccine developed by the Second Affiliated Hospital, School of Medicine, Zhejiang University. It is derived from tumor cells, such as those obtained from pleural effusions of patients with advanced non-small cell lung cancer (NSCLC). The production process involves treating tumor cells with the proteasome inhibitor bortezomib to induce the accumulation of short-lived proteins (SLiPs) and defective ribosomal products (DRiPs) within autophagosomes, termed DRibbles. These vesicles are then isolated and used as a vaccine to deliver a broad spectrum of tumor antigens, including neo-antigens and tumor-associated antigens, to dendritic cells. The vaccine specifically targets the CLEC9A receptor on dendritic cells to facilitate efficient antigen cross-presentation, thereby priming tumor-reactive CD8+ T cells. In clinical settings, it is frequently evaluated in combination with dendritic cell-cytokine-induced killer (DC-CIK) cell therapy to enhance anti-tumor immunity.
