Overview

Dronedarone is a class III antiarrhythmic small molecule developed by Sanofi-Aventis for the management of atrial fibrillation (AF) and atrial flutter (AFL). Chemically, it is a deiodinated benzofuran derivative of amiodarone, designed to reduce the adverse effects associated with amiodarone’s iodine content. Dronedarone acts as a multichannel blocker, inhibiting multiple cardiac ion channels including potassium (rapid delayed rectifier, slow delayed rectifier, ACh-activated inward rectifier), sodium (fast inward), and L-type calcium channels. It also antagonizes alpha and beta-adrenoceptors. Its primary clinical use is to maintain sinus rhythm and reduce hospitalization risk in patients with paroxysmal or persistent AF/AFL who have returned to normal heart rhythm. Dronedarone should not be used in patients with permanent AF or severe heart failure due to increased risk of mortality and cardiovascular events[2][4][5][9][10].
