**DS-1103a + trastuzumab deruxtecan** is an investigational combination therapy currently in early clinical evaluation for advanced solid tumors. DS-1103a is a recombinant humanized IgG4 monoclonal antibody targeting signal regulatory protein alpha (SIRPα), designed to block the SIRPα-CD47 immune checkpoint pathway and promote phagocytosis of tumor cells. Trastuzumab deruxtecan (T-DXd) is an antibody-drug conjugate (ADC) that combines a human monoclonal antibody against HER2 (human epidermal growth factor receptor 2) with a topoisomerase I inhibitor payload via a cleavable linker. This combination aims to maximize anti-tumor immune activity by unblocking macrophage phagocytosis (via SIRPα inhibition) and delivering targeted cytotoxicity to HER2-expressing or HER2-mutant cancer cells (via T-DXd’s ADC mechanism). The development is led by Daiichi Sankyo and involves collaborations with AstraZeneca.
