DS-9231 is a PEGylated recombinant staphylokinase variant developed by Daiichi Sankyo as a fibrin-selective thrombolytic agent. Staphylokinase, a protein derived from *Staphylococcus aureus*, acts by forming a 1:1 stoichiometric complex with plasminogen, which then converts free plasminogen into active plasmin primarily at the surface of a fibrin clot. This mechanism provides high fibrin selectivity, potentially reducing systemic fibrinogen depletion and the associated risk of major bleeding compared to non-selective thrombolytics. The addition of polyethylene glycol (PEG) chains to the recombinant protein is intended to extend its circulatory half-life and reduce potential immunogenicity. DS-9231 was primarily investigated in Phase 2 clinical trials for the treatment of intermediate-risk acute pulmonary embolism, though the program has since been discontinued from the developer's active pipeline.
