DSG3-CAART is an investigational, precision cellular immunotherapy designed for the treatment of mucosal pemphigus vulgaris (mPV), a rare and potentially fatal autoimmune blistering disease. It consists of autologous T cells engineered to express a chimeric autoantibody receptor (CAAR) that incorporates the extracellular domains 1–4 of desmoglein 3 (DSG3), fused to CD137-CD3ζ signaling domains. This design enables the modified T cells to specifically recognize and eliminate B cells that express pathogenic anti-DSG3 autoantibodies—the underlying cause of mPV—while sparing normal B cell immune function. The approach aims for antigen-specific B cell depletion, offering a targeted alternative to broad immunosuppression or pan-B-cell depletion therapies. Developed by Cabaletta Bio in collaboration with the University of Pennsylvania, DSG3-CAART has received FDA Orphan Drug and Fast Track Designations for pemphigus vulgaris[2][4][5][6][8].
