dsIL-18-Fc (also known as RG6312) is an engineered interleukin-18 (IL-18) cytokine fusion protein developed by Genentech (Roche) for the treatment of solid tumors. It is specifically designed to overcome the limitations of wild-type IL-18, which is rapidly neutralized by the endogenous decoy receptor IL-18 binding protein (IL-18BP) in the tumor microenvironment. dsIL-18-Fc is engineered to be resistant to IL-18BP while maintaining potent agonistic activity at the IL-18 receptor (IL-18Rα/β). The molecule consists of a modified human IL-18 sequence fused to an Fc scaffold to extend its half-life, and it incorporates an artificial disulfide bond (ds) to enhance structural stability and manufacturability. By signaling through the IL-18 receptor on NK cells and CD8+ T cells, dsIL-18-Fc stimulates the production of interferon-gamma (IFN-γ) and promotes a robust anti-tumor immune response, potentially turning immunologically "cold" tumors "hot."
