DT-216 is a novel small molecule developed by Design Therapeutics as part of its GeneTAC platform for the treatment of Friedreich's ataxia (FA). It is designed to specifically target the GAA repeat expansion mutation in the first intron of the FXN gene, which causes FA. The molecule contains a DNA-binding region that binds to the expanded GAA sequence and a second region that recruits cellular machinery to restore frataxin (FXN) gene expression. By unblocking transcriptional elongation, DT-216 increases production of functional FXN protein, addressing the root cause of FA. In Phase 1 clinical trials, DT-216 demonstrated dose-dependent increases in FXN mRNA levels and was generally well-tolerated, though injection site reactions led to reformulation efforts. The drug received FDA Fast Track designation for FA but development has shifted toward improved formulations due to tolerability concerns[1][3][4][5][6].
